Pennsylvania measles outbreak passes 940 cases as counties without health departments brace for spread
Pennsylvania has reported 943 confirmed measles cases across 39 counties and a fifth measles-associated death. Local officials told The Guardian that spread beyond the Lancaster County epicentre is likely, and that most of the state's 67 counties have no health department of their own.
Public health officials in Pennsylvania expect the state’s measles outbreak to keep spreading, The Guardian reported on 4 October 2026.
The numbers
- Last week the state reported its fifth measles-associated death, with 943 confirmed cases in 39 counties.
- Lancaster County has the most cases, 381, believed to be mostly among Amish and similar “Plain” communities, which historically have low uptake of the measles, mumps and rubella (MMR) vaccine.
- Chester County, next door, has the third-highest count, 83.
These figures are official case counts as relayed by The Guardian; the Pennsylvania health department had not commented at the time of publication.
A patchwork public-health system
Only 7 of Pennsylvania’s 67 counties have their own health department; the rest rely on the state. Lancaster County is one without its own agency, and local doctors and officials are now trying to create one.
Chester County health director Jeanne Franklin said her county is preparing for an influx of cases across its long border with Lancaster and assumes the virus may reach people outside the Plain community. She said many Amish residents are unvaccinated because they were unaware of the vaccine or seek health care less often, not because they oppose it, and that her nurses still hear myths — for example that the vaccine causes sterility or contains a microchip. Neither claim is true.
In Montgomery County, medical director Dr Richard Lorraine called cases there “a matter of time” and said the county is offering vaccination regardless of insurance status. Delaware County’s new health department said gaps in paediatric care after the bankruptcy-driven collapse of Crozer Health are a challenge.
Global picture
Measles is spreading elsewhere too. In Bangladesh, health-service data reported on 4 October put the tally since mid-March at 196,418 suspected cases (21,748 laboratory-confirmed) and 1,125 suspected or confirmed deaths.
Measles is one of the most contagious human viruses, and two doses of MMR vaccine are highly effective at preventing it. If you are unsure whether you or your children are protected, talk to your doctor or local health department.
