Smartwatch AI flags hypertension signs without cuff calibration in Nature study
A prospective multicenter Nature study of 196 adults without known CVD or antihypertensive drugs used consumer smartwatch PPG plus accelerometry, with no cuff calibration, against 24-hour ambulatory BP: over 7 days sensitivity was 65.8% and specificity 90.0% — framed as opportunistic screening, not a diagnostic replacement.
A prospective multicenter study published in Nature on 5 November 2026 evaluated an AI system on consumer smartwatch photoplethysmography (PPG) plus accelerometry, without cuff calibration, against 24-hour ambulatory blood pressure as the gold standard. The cohort was 196 adults without known cardiovascular disease or antihypertensive medication.
Performance
Over 7 days, sensitivity was 65.8% (95% CI 54.0–76.3), specificity 90.0% (83.2–94.7), and positive predictive value 80.6% (68.6–89.6). Against initial office BP, sensitivity was 55.3%, specificity 90.0%, and PPV 77.8%. Authors frame the approach as opportunistic screening to prompt clinical cuff confirmation — not a diagnostic replacement. Many authors are affiliated with Google Research.
Limits — talk to a doctor
The sample is small; many authors have industry affiliations. Screening metrics are not a diagnosis. This is not personal medical advice — talk to your doctor about blood pressure concerns.
