Survodutide Phase 3 SYNCHRONIZE-2: up to ~13.1% weight loss vs ~3.1% placebo in obesity+T2D
Boehringer Ingelheim’s dual GLP-1/glucagon agonist survodutide showed up to about 13.1% mean weight loss over 76 weeks versus about 3.1% on placebo in adults with overweight/obesity and type 2 diabetes (SYNCHRONIZE-2; NEJM + EASD). About 18% discontinued for GI adverse events vs ~1.2% placebo. Not FDA-approved; CVOT ongoing. Not medical advice.
Phase 3 SYNCHRONIZE-2 results for investigational survodutide, a weekly dual GLP-1 / glucagon receptor agonist from Boehringer Ingelheim (licensed from Zealand Pharma), were published 1 October 2026 in The New England Journal of Medicine and presented at the EASD meeting. Medical Daily and the company press release report that in adults with overweight or obesity and type 2 diabetes, survodutide produced up to about 13.1% mean body-weight loss over 76 weeks versus about 3.1% with placebo (company headline figure linked to the efficacy estimand, which assumes patients stay on treatment; intention-to-treat-style estimates are typically smaller).
Key efficacy and tolerability numbers
The trial enrolled about 755 adults randomised to weekly survodutide (3.6 mg or 6.0 mg) or placebo (NCT06066528). Up to about 79.3% on survodutide lost at least 5% of body weight versus about 32.7% on placebo. HbA1c fell by up to about 1.21 percentage points from a baseline near 7.4%, versus about 0.03 points on placebo. A notable drawback: about 18% discontinued because of gastrointestinal adverse events (nausea, vomiting, diarrhoea, constipation) versus about 1.2% on placebo, mostly during dose escalation.
Regulatory status and limits
Survodutide is not FDA-approved; no filing date was announced in the covered reports. The trial measured weight and glycaemia, not hard cardiovascular events; a separate cardiovascular outcomes trial (CVOT) is ongoing. Sponsor Boehringer stands to benefit commercially if approved. Cross-trial comparisons with semaglutide or tirzepatide are uncertain. This article summarises a sponsor-run Phase 3 RCT — not personal medical advice. Do not change diabetes or weight medicines based on these results; discuss approved options with a clinician.
